Juvenile Fibromyalgia Research Survey
Are you the parent or guardian of a child who has fibromyalgia symptoms?
The National Fibromyalgia Association is conducting a juvenile fibromyalgia (JFM) survey to better understand how this disorder impacts the quality of life of children. Results from this survey will be used by the NFA to develop resources specific to children with FM, and will also be shared with researchers to help them gain a better scientific understanding of this disorder. This survey is made possible by a grant from Forest Laboratories.
Survey participants must be at least 18 years old and be the parent or guardian of a child who has fibromyalgia symptoms such as body-wide chronic pain, fatigue, stiffness, sleep problems, headache, irritable bowel and restless legs.
The survey is anonymous and presents no foreseeable risks. There is no monetary compensation. The survey should take approximately 15 to 20 minutes to complete. If you have more than one child with FM or FM symptoms, please complete one survey for each child.
